How to Create a Free Entry Form:
Search for the Event: Find the event you're interested in through PerfDog's event search.
Create Entry Form: Click the "Create pre-filled entry form!" button in the Event status box.
Event Notice: You'll see a message stating that the event secretary isn't using PerfDog for online entries.
Follow Instructions: Continue through the online steps to create, print, and mail your pre-filled entry form.
